The result of this test must be negative, so the employer can be reassured of the candidate's current status. Last but not least, you must have the Driver File which must contain the following documents:


  • DOT Authorized Employment Application.

  • Background check format for previous jobs.

  • State MVR 3-year history.

  • Copy of the current CDL.

  • Copy of the current medical certificate.

  •  Annual review of driving record.

  •  Drug test.

  •  Certificate of receipt of alcohol and drug policy.

  •  Clearinghouse pre-employment search.
